About Us

Established in 1975 inside a small house on 8th Street, Neighborhood Health today serves more than 23,000 patients with more than 90,000 visits annually. We were the first Health Center in the state to achieve the highest level of recognition possible as a Patient-Centered Medical Home and we are accredited by the Joint Commission and Bureau of Primary Health Care.

Mission

Yakima Neighborhood Health Services’ mission is to improve the quality of life in our communities by providing accessible and integrated health and social services, ending homelessness, and offering unique learning opportunities for students of health professions.

Vision

Yakima Neighborhood Health Services strives to provide holistic health care in an environment free of discrimination, where safe and stable housing is available to all, and where staff feel rewarded for their efforts and achieve personal and professional growth in their work.

Values

Everyone has a fair and just opportunity to be as healthy as possible. This requires removing obstacles to health such as poverty, and its consequences, including powerlessness and lack of access to good jobs with fair pay, quality education and housing, safe environments, and health care.

This includes assessing clients’ needs, providing all required services, maintaining proper staffing levels, effective management, and operating accessible, safe, and affordable facilities.
  • All the five Neighborhood Health primary care locations have earned Level 3 Patient-Centered Medical Home Level 3 recognition. Neighborhood Health was the first Community Health Center in Washington state to be accorded the highest national level (Level 3) recognition – when the federal recognition program began.
